Shin Splints or Stress Fracture? Croydon Runners – Know the Difference


If you’re a runner in Croydon dealing with pain in your lower leg, you’ve probably wondered — Is this just shin splints, or something worse like a stress fracture?

It’s a common question, and an important one. Both injuries affect the shin and can feel similar early on, but knowing the difference is key when it comes to treatment, recovery time, and whether or not you can keep training.

Here’s how to tell the difference, what causes them, and when to get it checked.


Shin Splints (Medial Tibial Stress Syndrome)

This is the more common and less serious of the two. Shin splints usually come on when there’s been a sudden increase in running load — more distance, more hills, harder surfaces, or a return after time off.

Typical symptoms:

  • A dull, aching pain along the inner edge of your shinbone
  • Pain that often eases once warmed up, but can return after running
  • Tenderness along a broad area (not a specific spot)
  • Gets worse with more running, better with rest

Causes:

  • Rapid increase in volume or intensity
  • Poor footwear
  • Running on concrete or hard surfaces
  • Weak calves or poor foot biomechanics

What to do:

  • Reduce running temporarily (don’t stop completely unless pain is severe)
  • Strengthen your calves and foot muscles
  • Mix up surfaces (trail, grass, treadmill)
  • Use proper running-specific physiotherapy to correct load and technique

Stress Fracture (Tibia)

This is a more serious injury that often starts out like shin splints but progresses into something more localised and intense. It’s a small crack in the bone caused by overload over time — and it needs a break from running to heal properly.

Typical symptoms:

  • Sharp, pinpoint pain on one specific part of the shin
  • Gets worse the more you run
  • Pain doesn’t ease with warm-up and may linger or worsen afterward
  • Tenderness when pressing on one spot on the bone
  • May show swelling over the area

Causes:

  • Continuing to run through shin splints
  • Repetitive overuse without rest
  • Low bone density or poor recovery
  • History of under-fuelling or menstrual irregularities (especially in female athletes)

What to do:

  • Stop running and offload the limb — stress fractures need rest
  • See a physio for assessment (and possibly refer for imaging)
  • Start a return-to-run plan once cleared — not before
  • Address bone stress injury rehab factors like nutrition, load, and strength

When Should You Get It Checked?
